Athletic Policies
Participation on any one of SMC's intercollegiate teams or
athletic squads requires a full understanding and acceptance of the
the Athletic Department's Sports Medicine Policies. We urge all parents to read
and fully understand the Letter to Parents which pertains to their
student athlete's coverage while at Spartanburg Methodist
College.
In addition to all necessary admissions and academic enrollment
forms, there are several athletic forms that students wishing to
compete in intercollegiate activities at Spartanburg Methodist
College must complete and file with the SMC Athletics office.
Required forms:
Student athletes will not be permitted to practice and participate
in competition until all necessary forms have been filed and they
have been cleared by the SMC Athletics office.
- Each student athlete must sign and file the SMC Athlete Code of Conduct.
- Each student athlete must thoroughly review the SMC Athletics Drug Education and Testing Policy statement and sign and file the last page.
- The Physical Packet incudes a medical history and physical form, along with information on health care policies and acceptance of risk. If the student athlete is under the age of 18, the treatment and consent form must be signed by their parent or guardian. All parts of this packet must be filled out entirely, signed, and filed with the SMC Athletic Department before the student athlete will be allowed to practice and participate in competition.
Student athletes are personally responsible for completing these
forms each year and delivering the original documents in person to
the SMC Athletic Department during registration. Please do not mail
or fax copies of these forms.

This form must be filled out and submitted to the SMC Athletic Office prior to trying out for any sport or leadership squad.
